Be Prepared for Medical Emergencies Involving your Child Brochure

This brochure will give you the basic information you need to be prepared for medical emergencies with your child.  A special section is included for children with special needs.

Premises Warning Form (Coming Soon)

If your child has a serious medical condition that may require a special EMS response; fill out this form, have it signed by your child's physician and return it to BVA, Attn.: Premises Warning Forms.   BVA will have the information entered into the Monroe County 9-1-1 dispatch computer.  Any emergency agency responding to your address will be alerted to your child's condition.

Emergency Information for Children with Special Health Needs

This important document, properly filled out, will help to assure prompt and appropriate care for Children with Special Health Care Needs. The EIF will ensure that a child's complicated medical history is concisely summarized and available when it is needed most - when the child presents with an acute health problem at a time when neither parent nor pediatrician is immediately available.

Personal Medical History

If you or a member of your family has a medical emergency, the physician will need the patient's medical history. Simply print and fill out (and don't forget to update) one of these forms for each member of your family. Make sure you keep it in an easily accessible place so that if there is an emergency, you will be know where it is and will be able to bring it to the hospital or physician's office. You should also keep a copy with you when traveling.

Consent to Treat Form

This Consent to Treat Form gives a physician permission to treat your child when he or she is in someone else's care.
